Gianluigi Forloni

Gianluigi Forloni is head of the Neuroscience Department at the Istituto di Ricerche Farmacologiche Mario Negri IRCCS in Milan, a department that includes a prion neurobiology laboratory. He is the scientific lead of the long running Italian preventive trial of doxycycline in people at genetic risk of fatal familial insomnia, which the Mario Negri Institute sponsored with Fondazione Telethon funding, and he was second author of the Italian and French randomised doxycycline trial in CJD published in 2014.

Key facts

  • The Mario Negri Institute department page, retrieved 2026-10-16, names him Capo Dipartimento (head) of the Neuroscience Department, and every recent paper read lists his affiliation as Department of Neuroscience, Mario Negri IRCCS, Milan.
  • An editorial board biography seen only as a search excerpt says he has headed the Biology of Neurodegenerative Diseases Lab since 1996 and the Neuroscience Department since 2002, after a biology degree in Milan in 1985 and postdoctoral work at Johns Hopkins; a 2012 blog post gives the same two roles.
  • His department hosts the Laboratorio Neurobiologia dei Prioni and lists four prion projects: DOXIFF funded by Fondazione Telethon, ProFFIle funded by the EU Joint Programme Neurodegenerative Disease Research, an anti-prion compound study under Ministry of Health grant RF-2016-02362950, and a CJD Foundation funded study of prion vaccines in transgenic mouse models of familial prion disease.
  • The same trial is registered as NCT04846335, first posted only in April 2021: phase 2, sponsor Mario Negri, collaborator Fondazione Telethon, start 28 April 2011, primary completion 12 April 2021, completion 30 November 2024, 29 enrolled, primary outcome survival at ten years, and no results posted.
  • He is first author of the 2015 Prion paper that set out the design: doxycycline 100 mg a day for ten years in asymptomatic carriers over 42, judged against historical incidence because a randomised controlled trial was considered ethically unfeasible, with Telethon grant GGP10208.
  • He is first author of the 2022 Prion paper on public engagement, written with AFIFF members, Treviso clinicians, Carlo Besta neurologists and a Telethon officer, which explains that genotype stayed blind to participants and lists four Telethon grants (GGP10208, GSP14001, GSP18001, GSP21002).
  • He was second author of the phase 2 randomised placebo-controlled doxycycline trial in 121 CJD patients in Italy and France (Lancet Neurology 2014), with Maurizio Pocchiari, Inga Zerr and Fabrizio Tagliavini among co-authors; it found no survival benefit.
  • He co-authored the 2024 Nature Reviews Disease Primers overview of CJD and other prion diseases with Inga Zerr, Anna Ladogana, Simon Mead, Peter Hermann and Brian Appleby.
  • His 2019 review of prion therapeutic trials argued that sensitive detection of prion aggregates for early biomarkers and immunotherapy research are the openings for prevention.
  • In the last 24 months his indexed prion related output is laboratory work on the cellular prion protein as a mediator of tau oligomer toxicity (2025) and a sole author 2025 review of doxycycline in Alzheimer disease; no trial outcome paper was found.
